Supervision Session Prep

Structured Supervision-Session Prep
Step 1 of 5

About this tool

This prep sheet pulls together what you already know about a case — from the Therapist Alliance & Process Check-Up (short or long form) and, if your client completed it, the client-facing Therapy Check-Up — into a single, organized agenda you can bring to your next supervision session.

You’ll be asked which of those tools were completed and what stood out. Areas this tool helps you prepare:

1
Case snapshot & context
2
Risk & safety status
3
Alliance flags (therapist-side)
4
Client feedback flags
5
Case conceptualization questions
6
Countertransference / self-of-therapist
7
Ethical / scope questions
8
Your goal for this session

Every field is optional — fill in what applies to this case and skip the rest. If a tool wasn’t completed, say so; the agenda will note it as a gap rather than skipping it silently.

Nothing is saved. This tool is not a validated clinical instrument and does not store, transmit, or retain anything you enter. Print or save your agenda as a PDF before closing the page.
Step 1 of 5

Case Snapshot

A quick orientation for your supervisor before you get into specifics.

Client identifier (optional)
Initials, case number, or a pseudonym — nothing is saved.
Why are you bringing this case to supervision?
A sentence or two of context is enough.
What do you want to walk away with from this session?
A clear question or outcome keeps the discussion focused.
Step 2 of 5

Risk & Safety Status

Anything current that should be flagged up front, before the alliance discussion.

Are there any current risk or safety concerns to flag?
Briefly describe — this will be placed first in your printed agenda.
Step 3 of 5

Therapist Alliance & Process Check-Up

Pull in whatever stood out when you last completed this for this client.

Did you complete the Therapist Alliance & Process Check-Up for this client?
For each domain, flag how it came back:
Step 4 of 5

Client’s Therapy Check-Up

If your client completed the client-facing check-up, bring their perspective into the agenda too.

Did your client complete the Therapy Check-Up?
For each area, flag how the client’s feedback came back:
No problem — the agenda will note the client hasn’t completed this yet, which can itself be worth raising in supervision.
Step 5 of 5

Additional Prep

Anything else worth bringing — all optional.

Clinical or case conceptualization questions
Countertransference / self-of-therapist notes
Ethical or scope-of-practice questions
Follow-up from your last supervision session
